How do I fill out my Online Polling Form in VinMotion?

After the local DS Rep orders Polling service for a dealership, the master account-holder (Administrator) at the dealership needs to fill out the Online Polling Form(OPF). This form provides us with the details of the dealer's DMS setup and tells us what to poll & how to access the information. The form is located in VinMotion under System Preferences -> Settings - > ... -> Polling. If the user doesn't see the option, either the service isn't ordered yet or the user doesn't have an account with permissions to edit the form.
