How can a dealership sign up for Dynamic Window Labels on the Carfax site?

For a dealer to get set up with Carfax dynamic window stickers, the dealer must call Carfax to request an authorization form. The number is 888-788-7715. The completed form should be emailed to winstickplus@carfax.com or faxed to 703-218-2853. (update 5/8/13)
